As for last names, I wish I had the same last name as my fiance, it would make this figuring out what to change our names to thing easier. My last name's Adams and one of my girlfriend's grandparent's last names is Adam, so some light background checking went on. If you aren't related, then who cares. I don't think having the same last name is a big deal.
Even if you are related, who cares? The grandparents having your last name would be pretty distant. It would be silly to end a relationship just because you found out that your girlfriend's grandfather was your grandmother's brother in law or something like that.

If I found out we shared a great-grandparent or something like that though, I'd definitely reconsider. It would make more sense to do it to avoid getting made fun of or being the brunt of jokes than to worry about genetic issues. Why would it be weird or taboo. It just makes things easier actually. No name change necessary assuming marriage is a possibility.
